Online Enrollment Grades 1-12

The paperwork portion of registration is completed online.  If you have questions or do not have access to a computer, please contact Mrs. Carol Staley at [email protected] or 724.796.1551 ext 2418.

Below is a step-by-step guide for registering your child for Grades 1-12 (New and Returning Students)

Step One:

To get started, the District requires the following information to complete your child's registration.

Please begin by gathering the items listed below:

-Your child's birth certificate
-Two Proofs of residency (lease/rental agreement/mortgage statement; automobile registration/insurance card; utility bill; pay stub/ACCESS card)
-Photo ID of Parent/Guardian enrolling child (Driver's License, Passport or Identify Card - cannot be expired)
-Copy of your child's YTD Immunizations as required by law (must be a copy from Health Care Provider)

You will be asked to upload these documents...a clear scan or picture (jpeg or the like) will upload well.

Step Two:

Complete the online registration packet.

You will begin by creating a login on our Student Registration Community Web Portal.


(1) Click on CREATE a NEW ACCOUNT--you will be asked to enter your first and last name; working email address; desired password and a security question/answer.

(2) Once you complete this step, you will be directed to verify your email address.  Check your email for a message titled "Community Web Portal Student Registration Account Email Confirmation".

(3) Verify your newly created account through the email sent to you.  You are now ready to begin the online registration process.  (It is very important to remember your newly created password--you will need to enter your email and password each time you enter the registration portal.)

If able, please scan the birth certificate, verification of residency, photo ID, immunizations, and health forms.  You may take a clear picture with your phone or use the scan option in the notes app to create an image of these documents for uploading.

Step Three:

The District requires that several documents need to be downloaded, completed, and returned to the District.  Hard copies of these documents can be requested if you cannot print them at home.

(You may return these documents by scanning and attaching them to an email, sending them through US mail, or dropping them off to our District Offices.)

HEALTH FORMS:

Return the following completed health forms to Mrs. Ashley Barghausen, Elementary Center Nurse (grades 1-6), [email protected] and/or Mrs. Rebecca Cecchini, High School Nurse (grades 7-12) [email protected]:

     1. Physical Examination form is required for students in 6th and 11th grade (form provided in the online registration packet, must be completed by your Pediatrician/Family Physician)
     2. Dental Examination form is required for students in 3rd and 7th grade (form provided in the online registration packet, must be completed by your Dentist)
     3. Scoliosis screening is required in 7th grade.

If your child has already seen a doctor or dentist IN THIS CALENDAR YEAR, please complete the parent portion, then have the medical professional complete the rest of the form and upload it to the online registration portal.

RESIDENCY FORMS (forms only need to be completed if you answer "YES" to either question below):

     1. Are you a parent/guardian who is renting/leasing without a written agreement? (No Lease Agreement Form will need to be completed).
     2. Are you a parent/guardian who is living with a Fort Cherry Resident in a multiple occupancy situation? (Proof of Residency Form will need to be completed).

Forms may be downloaded from the online registration packet.

Return the following completed forms to Mrs. Carol Staley, District Registrar, [email protected].

Step Four:

Come to the Administration Office (behind the Elementary Center) to complete the 2 forms that need to be notarized (the district has free notarization) and sign a request for records so that we may ask the prior school for your student's records.

Summer Hours:  Monday through Thursday, 9:00 am - 3:00 pm. The office is closed on Fridays.
